​Exciting progress made on the Acute Coronary Syndrome pathway


I attended an event at St George's hospital where the cardiology department presented work they have been doing on the Acute Coronary Syndrome pathway. The team have worked intensively for the last three months to review and redesign the patient pathway to integrate care better across the hospital departments and between the inpatient and community service.

Wandsworth CCG has been particularly involved in the new continuing cardiac care service (previously known as cardiac rehabilitation). The new CCC service follows national guidance on delivery of this highly effective intervention. The evidence base for cardiac rehab is very strong and patients who attend are significantly less likely to have a further cardiac event.

Details of the new service will be available soon, but meanwhile GPs are encouraged to use the existing cardiac rehab pathways for patients who need secondary prevention and have not previously attended.
